Each contest coordinator (publication or broadcaster) or person entering as an individual must fill out one copy of the Cover Letter (click here). This identifies who this group of entries is coming from, whom the contact person is, and how the contact can be reached. It also includes the total number of entries from this source and the amount of the check. This form should be included in the box or envelope along with the check.

Then each entry must be accompanied by a copy of the Entry Form (click here). This tells us the name and category of the entry, how the entrants are to be named if the entry wins, and the amount paid for the individual entry. A form is to be attached to each entry.

Thus, a newspaper entering 25 stories and photos would need one Cover Letter and 25 Entry Forms. A member entering one story as an individual would need one Cover Letter and one Entry Form.

In addition, broadcast entrants must attach this coupon (click here) to each cassette or disc entered. The coupon need not be used if the exact information is attached to each cassette or disc in some other manner.
